Supporting social referrals and resources

More and deeper community partnerships improved the reach of Humana’s Bold Goal in Chicago in 2019. With several strong provider relationships, a social health referral platform gained adoption and is being used to improve the connection between community resources and members in need of support. Humana is participating in the Healthy Chicago 2025 Taskforce and has supported the City of Chicago Farmers Market Partnership as well as the Greater Chicago Food Depository, which counts 700 food pantries, soup kitchens, shelters and support programs among its network in Cook County.
