Gaining partnerships and momentum
In 2019, Humana spent time introducing itself and working with more community partners. More than 40 partners are now collaborating on meeting social determinants of health needs. These organizations are forging a pathway for health care and human services to be more integrated and gain insight into needs for our shared community members. Of particular note is Humana’s new partnership with the National Kidney Foundation that helped to host a Kidney Early Evaluation Program (KEEP) screening (risk assessment for kidney disease) as part of a local wellness expo.
