We help to provide whole-person health by building and maintaining vital strategic partnerships, including:
Partnerships at the national level to address health-related social needs and help achieve health equity for our members and communities at large.
Partnerships at the local/city level to address barriers to healthy living, reduce disparities and improve the health of Humana’s diverse communities.
Partnerships with physicians, clinicians and Humana care teams to integrate health-related social needs into our clinical, home health and pharmacy capabilities, and enable them to deliver optimal and equitable care.
Partnerships with universities and other academic institutions to train and educate the clinicians of the future on addressing health-related social needs and expanding the use of value-based payment models.
